地點：S101教室 (體育館1樓)

時間：9:00AM-4:00PM

邀請大家一起關心自己健康，自費項目可參考【附件1】、【附件2】。

＊109學年度新生及109年2月新生注意事項:

1.(1)線上看報告者有複檢單畫面顯示，請自行印出帶至體檢現場，可免費複檢。

  (2)領取紙本報告者內附複檢單，持複檢單可免費複檢。

2.請帶學生證備查。   

3.請先到體育館1樓川堂(7-11旁)報到

4.請戴口罩前來體檢

5.因應防疫需求，體檢現場將實施排隊距離管制

For your health,we organized Medical checkup recheck activities,expense items can refer [Annex 1], [Annex 2].

If you are 2020.Sep(or 2020. February) new student several abnormal findings in health examination result ,

please remember to bring medical examination review form (for free) and come to Health Services Division on the following scheduled dates.

Note:

1. Please remember to bring your medical examination review form and your student card.(If you can't find form,you can take your student card.)

2. Please report to the gym hallway on the first floor (between Health Center and 7-eleven).

3.Please wear a mask to come to the medical examination (review).

4.To meet the needs of epidemic prevention,queuing distance control will be implemented at the medical examination site.
