📍申請表須提供紙本資料與電子檔案，紙本資料請提交至雙語辦公室(醫揚大樓10樓 IA-1006)，電子檔案請上傳至本表單！
📍紙本申請表之署名表請詳閱後親簽。社群成員由校內同學依序親簽，完成後再繳交至雙語辦公室。期間，電子檔仍可先行上傳。
⚠️上傳電子檔案時請注意以下事項：
🔹檔案上傳格式請使用文件檔(WORD)
🔹申請表檔名格式：社群名稱_召集人姓名，如：跨領域EMI課程增能社群_麻吉兔
🔹表單提交成功後，您將於登入的信箱位置收到一份「回覆副本」

【Event Details】
• Registration Deadline: 2026 / 11 / 6 (Fri)
• Team Composition: Teams must consist of 4 to 8 students, comprising at least 2 different nationalities.
• Eligibility: Must hold an EMI TA certification, or be a current or former EMI TA.
** This semester, we are also accepting applications from students who do not yet hold the EMI TA certification but are expected to obtain it. **

• Important Notes:
📍 Both paper and electronic applications are required. Please deliver the paper copies to the Office of Bilingual Education Initiatives (Room IA-1006, 10F, Yiyang Building) and upload the electronic file to this form!
📍 Please review and personally sign the signature page of the paper application. Student members must sign in sequence before submitting it to the office. The electronic file can still be uploaded beforehand.
⚠️ Please note the following when uploading your electronic file:

🔹 File Naming Format: Community Name_Convener Name (e.g., Interdisciplinary EMI Empowerment Community_Donald Trump)
